Planned support

A support model built with the resident.

Navera Health and Wellness is developing a model for coordinated services, coaching and help with daily living alongside Meridian’s residential offering.

A community in development

Support services

Professional care

Support services would be delivered or coordinated by appropriately qualified professionals. Assessment and treatment would use a separate support process.

  • Care based on individual needs
  • Coordination with the person’s other providers
  • Separate registration and information-sharing permissions
About support service inquiries

Coaching

Practical daily support

Coaching could focus on chosen goals, routines and participation. Any assessment or treatment would be arranged through qualified providers.

  • Executive functioning and learning
  • Household and daily living skills
  • Community and social participation
An illustrative week

The opening service scope, staffing, hours and response arrangements are still being defined. This website does not promise current service availability or continuous crisis coverage.

Planning with the resident

Start with what matters
to the person.

Family involvement would reflect the adult resident’s preferences and applicable authorization.

  1. Discuss priorities

Understand the resident’s goals, routines and preferences.

  2. Choose useful supports

Identify the services and practical help that could support those goals.

  3. Agree on the arrangements

Explain responsibilities, fees and how information would be shared.

  4. Review progress

Agree on review points and adjust the plan with the resident.

Housing and services

Separate agreements.
Clear expectations.

Housing and Navera services would be arranged and billed separately. Available services, fees and payment options would be explained before enrollment.
